CURRENT OPEN POSITIONS

ADMINISTRATIVE ASSISTANT-ENRICHMENT PROGRAMS
Location:  Restorative & West Valley Life Enrichment Programs-Peoria/Sun City, AZ       

Hours: Part-Time 24 hours per week    

(Hours will be divided between RLEP & WV. This position may be combined with Life Enrichment Partner 16 hours per week position)

Qualifications/Requirements:
*4 years administrative/clerical experience- required.

* Demonstrated ability to maintain high level of confidentiality and customer service- required.
*High School Diploma or GED-required.

*Fingerprint Clearance Card, CPR/First Aid Certification, Food Handler’s Card
*Valid driver’s license and 21 years of age (company vehicle driving requirement)

Position Summary:
The purpose of the position is to support the Life Enrichment Program team with duties and tasks at the Program. This position will be the support to the Life Enrichment Team to ensure the Program is meeting all of the
regulations for CARF, AAA, VA, DDD, DHS, CACFP, and Environmental Services.

HEALTH & WELLNESS SPECIALIST, RN
Location:  All Programs- Peoria, AZ, Surprise, Sun City, AZ                       

Hours: On-Call (0-19 hours per week)

Qualifications/Requirements:
*2 years nursing home, public health or home care experience

*Associates Degree in nursing

*Current Arizona R.N. license, or meets requirements of Arizona RN licensure as regulated by the Nurse Licensure Compact

*Fingerprint Clearance Card, CPR/First Aid Certification, Food Handler’s Card
*Valid driver’s license and 21 years of age (company vehicle driving requirement)

Position Summary:

The Health and Wellness Specialist, RN provides engaging person directed programming, which includes implementing relationship-based meaningful individualized activities, providing choices, and encouraging the
members to be as independent as possible. This position provides assistance to the Director of Life Enrichment as a member of a multi-disciplinary team. Assesses members’ health status, provide nursing services,
and provides health and wellness counseling/education necessary to meet each member’s individual needs. The Health and Wellness Specialist also assists in enhancing the daily lives of members and supporting
their continued involvement in the community.

LIFE ENRICHMENT PARTNER
Location:  Mary’s Place                                                             

Hours: Part-Time 16 hours per week     (May be combined with Administrative Assistant-Programs 24 hours per week position)

Location:  Restorative Life Enrichment Program               

Hours: Part-Time 30-39 hours per week

*1 year working with diverse population of adults with developmental disabilities in day
program or group home environment

*Fingerprint Clearance Card, CPR/First Aid Certification, Food Handler’s Card
*Valid driver’s license and 21 years of age (company vehicle driving requirement)

Position Summary:

The Enrichment Partner position provides engaging person directed programing, which includes relationship-based meaningful and individualized activities, providing choices, and encouraging the members
to be as independent as possible. This position receives supervision from the Director/Coordinator of Life Enrichment. This position is an essential part of the interdisciplinary team and assists with enriching
the daily lives of the members.

TRANSPORTATION SPECIALIST_ BUS/VAN DRIVER
Location:  All Programs-Peoria, Surprise, Sun City, AZ    

Hours: On-Call   

(0-19 hours per week- May be combined with GSE Job Coach 20-29 hrs week position or Life Enrichment Partner 16 hrs week position)
Qualifications/Requirements:
*1 year providing transportation (bus/van driver) to a diverse population of adults with developmental disabilities

* Excellent verbal and written communication skills

* Valid Arizona Driver’s License with clean driving record for past 5 years.

*Fingerprint Clearance Card, CPR/First Aid Certification, Food Handler’s Card
*Valid driver’s license and 21 years of age (company vehicle driving requirement)

Position Summary:

The Transportation Specialist assists in providing Person Directed Care which includes providing choices, and encouraging the members to be as independent as possible. The Transportation Specialist
is responsible for transporting members to and from the program and to other program activities, in a safe manner.  The Transportation Specialist is also responsible for monitoring the vehicle daily for
operational and maintenance concerns, and reports to the Director of Enrichment appropriately.

GSE JOB COACH
Location:  Helping Partners Program-Sun City, AZ                                            

Hours: Part Time (20-29 hours per week)


Qualifications/Requirements:
*3 years working with diverse population of adults with developmental disabilities in day program or group home environment

*Valid driver’s license and 21 years of age (company vehicle driving requirement)

Position Summary:

The Job Coach position provides engaging person directed group supported employment and Life Enrichment programing, which includes relationship-based meaningful and individualized activities, providing choices,
and encouraging the members to be as independent as possible. This position receives supervision from the Director/Coordinator of Life Enrichment. This position is an essential part of the interdisciplinary team and
assists with enriching the daily lives of the members.

FAMILY SUPPORT COORDINATOR
Location: FRC-Surprise, AZ (Surprise Resource Center)                         

Hours: Full Time (40 hours per week)

Qualifications/Requirements:
*1 year experience with community services and resource knowledge of services for children and families.
* BA/BS degree in either early childhood development/education, family services, social work or closely related field-required.
*Fluent in reading, writing in English and Spanish-required.
*Computer skills (MS Applications) and knowledge of data processing programs.
*Must have reliable transportation, valid driver’s license and be able to drive to various community locations.
*Must possess the ability to maintain a high level of confidentiality and ability to work and travel independently.

Position Summary:

The purpose of the FSC is to allow families to receive individualized case management services including an assessment of their strengths and needs, a service plan, family goals, assistance connecting to identified services,
and follow up to ensure successful enrollment in key services.  The position requires the ability to collaborate with other agencies and to create working relationships with groups to increase the awareness of resources for
families in the Region.  This position is a 12-month, grant funded position through First Things First.
